A select group of people are chosen to perform an exercise which supposedly decreases ones blood pressure. If half of the people get a lower blood pressure and the other half don’t, does this suggest causation or correlation.

It suggests correlation. Since only half of the people got a lower blood pressure, this means the exercise had nothing to do with the results and did not cause the outcomes.
