ED Thoracotomy Indications
Blunt trauma and arrested <10 min before arrival at ED
Penetrating trauma and arrested <15 min before arrival at ED
ED Thoracotomy Management/ Steps
1- Call surgery
2- Incision at fifth intercostal space
3- Cut from sternum to posterior axillary line
4- Blunt dissect into pleural space
5- Rib spreaders
6- Cross clamp aorta
7- Open pericardium parallel to phrenic nerve
8- Repair heart
Consider surgery if ______ bleeding is present in Hemothorax
Initial drainage >1.5 L
Ongoing bleeding >200 mL/h for 2-4 h
Management of open pneumothorax (“sucking chest wound”)
Cover with occlusive dressing on 3 of 4 sides
Watch for tension pneumothorax
Place chest tube at separate site
