when does lower esophageal sphincter open during a normal swallow?
shortly after initiation of swallow
esophagus is innervated by
vagus nerve (dorsal motor nucleus and nucleus ambiguus)
is smooth muscle inhibition greater or less as you move down the esophagus?

2 broad etiologies for disordered esophageal phase of swallowing
structural abnormality
abnormal motility
globus sensation
sensation of a lump in the throat, does not actually interfere w/ swallowing though
dysphagia + odynophagia
most likely due to
infection
most important consequence of oropharyngeal swallowing problems
aspiration - may have to make NPO if this risk is too high and feed w/ NG tube or something
dx: intermittent dysphagia for solids only
esophageal ring or eosinophilic esophagitis
dx: progressive dysphagia for solids only + chronic heartburn
peptic stricture
dx: progressive dysphagia for solids only + older age + wt loss
esophageal malignancy
dx: intermittent dysphagia for solids and liquids + chest pain
diffuse esophageal spasm
dx: progressive dysphagia for solids and liquids + chronic heartburn
scleroderma
dx: progressive dysphagia for solids and liquids + bland regurg, wt loss
achalasia
