Sodium bicarbonate MOA
Dissociates to provide bicarbonate ion which neutralizes ion concentration and raises blood and urinary pH
Sodium bicarbonate indications
metabolic acidosis, low serum sodium
Sodium bicarbonate classification
Alkalinizing agent
Sodium bicarbonate is given
PO
You never give sodium bicarbonate IV for
hyponatremia
If sodium bicarbonate is being given IV push then
it would be for metabolic acidosis
Vesicant
kill the cells around it
If giving sodium bicarbonate PO then give it
1-3 hours after or before meals, it will be better absorbed that way
What to monitor when giving sodium bicarbonate
cardiac, ABGs, and electrolytes
If giving sodium bicarbonate IV
monitor IV potency thoroughly!
Major adverse effects of sodium bicarbonate
Edema, cerebral hemorrhage, hypernatremia, electrolyte abnormalities, metabolic alkalosis, flatulence with PO, tetany, pulmonary edema, heart failure exacerbation
