7 - Complete independance
6 - Modified independence (device)

4 - Minimal Assist (subject = 75%+)
3 - Moderate assist (subject = 50% - 74%)

2 - Maximal Assist (subject = 25% - 49%)
1 - Total Assist (subject < 25%)
Rating scale for eating
7 - patient performs all tasks
6 - patient uses adaptive equipment
5 - patient requires supervision or setup from helper
4 - patient performs 75%, helper physically assists picking up utensils or checks for food pocketing
3 - patient performs 50 - 74%, helper scoops food on spoon and client takes spoon to mouth
2 - patient performs 25 - 49%, helper scoops food and puts spoon in patients mouth
1 - patient performs less than 25%, only chews and swallows or relies on feeding tubes
Rating scale for grooming
7 - pt performs all tasks
6 - patient uses adaptive equipment/safety considerations, or takes a lot of time
5 - patient requires supervision or set up help
4 - patient performs 4/5 or 3/4 tasks themselves
3 - patient performs 3/5 or 2/4 tasks
