Racemic EPI
Racemic EPI Bronchodilator, sympathomimetic
Brand: Vaponefrin, MicroNEFRIN
MOA: marked stimulation of alpha, beta-1 and beta-2 receptors leading to bronchodilation and decongestion
IND: Croup, asthma, bronchospasms due to COPD
Con: epiglottitis, narrow angle glaucoma, use cautiously with pt’s who are hypertensive or tachycardia, cardiac disease, hyperthyroidism
Adverse: restlessness, tremors, anxiety, dizziness, headache, tachycardia, hypertension, palpations, angina
Dose: Adult- 0.25-0.75 ml of 2.25% racemic epic diluted in 3ml NS every 1-4 hrs
Pedi: same as adult
Specials: monitor vitals, Preggo C
Tylox
Oxycodone-acetaminophen OPIATE PAIN RELIEVER
Brand: Tylox
Oxycodone: inhibits ascending pain pathways, alterations in response to pain
Acetaminophen: non opooid analgesic, works peripherally to block pain. Acts on hypothalamus to produce antipyretic
IND: Pain
CON: Hypersensitivity, asthma, resp depression
Adverse: respiratory depression, hypotension, increased ICP, hepatic/renal impairment, N/V, sedation, euphoria,
DOSE/ROUTE: Codone 2.5-5mg PO Acetominophen 325mg Pedi=NO Special notes: use w/ caution in head injuries, max of 4000mg/day. Prolonged use can lead to addictions Prego class C
Ibuprofen
Ibuprofen NSAID
Brand: Motrin, advil
MOA: Inhibits prostaglandin synthetase causing anti-inflammatory and analgesic effects
IND: Mild-moderate pain, fever, arthritis, osteoarthritis,
CON: hypersensitivity, asthma, hepatic disease
Adverse Reactions: Dizziness, heartburn, N/V, rash, constipation
Dose/Route: Adult 20-800mg PO
Pedi 5-10 mg/kg PO
Special Notes: Preggo class C
Vicodin
Hydrocodone-acetaminophen Opiate Pain Reliever
Brand: Vicodin
MOA: hydrocodone= binds to opiate receptors resulting in analgesia, acetaminophen produces antipyresis. Inhibits prostaglandin synthetase
IND: pain
CON: Hypersensitivity
Adverse: respiratory depression, hypotension, increased ICP, hepatic/renal impairment, N/V, sedation, euphoria, Addisons
Dose/Route: Hydrocodone 5-7.5 mg PO
Acetaminophen 500-750mg PO
Pedi: not indicated
Special: do not use in conjunction with alcohol, preggo C
