Week 10 Flashcards
Calcium gluconate
Calcium gluconate Electrolyte
MOA: increase cardiac contractility & automaticity. Stabilize cardiac cells
IND: Hyperkalemia, hypocalcemia, calcium channel block , antidote for Mag, abdominal spasms due to Portuguese Man bites & spiders
CON: Dig patients, sensitivity, Hypokalmeia, Hypercalcemia
Adverse: hypotension
Dose/Route: 15-30ml(1.5-3)g over 10 min IV
Pedi: 60-100 mg/kg
Special: line should be flushed before bi-carb provided. Non-necrotic, less bioavailability.
Calcium Chloride
calcium chloride Electrolyte
MOA: increases calcium, increase cardiac contractility & automaticity. Stabilize cardiac cells
IND: Hyperkalemia, hypocalcemia, Calcium channel blocker OD, antidote for Mag, abdominal spasms due to Portuguese Man of war bites & spiders
CON: Dig patients, sensitivity, Hypokalmeia, Hypercalcemia, acute MI
Adverse: hypotesion, extravasation = necrosis, N/V, bradycardia, syncope
Dose/Route: 2-4mg/kg 10% solution IV
Pedi: 5-7mg/kg 10% solution IV
Special: line should be flushed before bi-carb provided, chalks from sodium bicarb and interacts with EPI, Necrotic, prego C
Dobutamine
dobutamine Sympathomimetic
Brand: Dobutrex
MOA: Beta 1 selective adrenergic receptors; increase cardiac contractility, with little chronotropic activity
IND: decreases cardiac output due to M.I., bradydysrhythmia with heart block Cardiogenic shock, CHF
CON: hypovolemia, sulfa sensitivity
Adverse: headache, HTN, palpitations, dysrhythmias, nausea, hypotension.
Dose/Route: 2-20 mcg/kg/min drip IV/IO
Pedi—same
Special: line should be flushed before bi-carb provided, prego class B
Furosemide
furosemide Loop Diuretic
Brand: Lasix
MOA: decrease preload and fluid. Reabsorption of NaCl, diuresis, vasodilation. Works in the loop of henley
IND: CHF, Pulmonary Edema
CON: dehydration, sensitivity or hypersensitivity to sulfa, pregnancy, dehydration
Adverse: hypotension, K+ depletion, n/v, headache, dysrhythmia, dehydration
Dose/Route: 40-80mg PO/IV/IO
Twice the pt’s dose
Pedi: 0.5-2mg/kg slow push IV/IO
Special: Photosensitive, if pushed too fast can cause transient or permanent hearing lose
