Insulin
Insulin Hormone
Brand: Humilin, Novolin
MOA: Causes uptake of glucose by cells, decreases blood glucose level, promotes glucose storage
IND: DKA, Hyperglycemia
CON: Avoid overcompensation, hypoglycemia
Adverse: Hypoglycemia, few in emergent setting
Dose/Route: IV/SQ Dose dependent on BGL
Special Notes: excessive dose may cause hypoglycemia
Glucagon
Glucagon Hormone
MOA: causes breakdown of glycogen to glucose, inhibits glycogen synthesis over all increasing: blood glucose level, cardiac contractile force, HR
IND: Hypoglycemia, Beta Blocker OD,
CON: hypersensitivity
Adverse Reactions: hyperglycemia
Dose/Route: 3mg IM/IV/IO
Beta Block OD: 5-10mg IM/IV/IO
Pedi= 0.03mg/kg IM/IV/IO
Special Notes: Not to be used instead of D50, Preggo B, companies have “controversial issues” with it because its $150/dose
Sildenafil
Sildenafil Phosphodiesterase Inhibitor
Brand: Viagra
MOA: increases cGmp, allowing smooth muscle relaxation
IND: ED, pulmonary HTN
CON: use of nitrates, hypersensitivity
Adverse: headache, flushing, priaprism, nausea
Dose/Route: 25-100mg PO or Daily
Special Notes: Preggo B
Pradaxa
Dabigatran Anti-Coagulant
Brand: Pradaxa
MOA: Directly/reversibly inhibits thrombin
IND: Atrial Fibrillation
CON: Active bleeding, hypersensitiity, Renal failure
Adverse: GI bleed, ulcer, thrombolytiopenia, angioedema,
Dose/Route: 150mg PO
Pedi: NONE
Special Notes: Preggo C, not reversible
Cialis
Tadalafil Phosphodiesterase Inhibitor
Brand: Cialis
MOA: smooth muscle relaxant & relaxation of pulmonary smooth muscle, increasing blood flow to the corpous cavernosum
IND: ED, benign prostate hyperplasia, pulmonary HTN, renal/hepatic impairment
CON: use of nitrates, dermatitis, hypersensitivity
Adverse Reactions: headache, flushing, priaprism, nausea
Dose/Route: 2-20mg PO or Daily
Special Notes: Preggo B, interacts w/ BP
Glucose
Glucose Carbohydrate
Brand: Glutose, insta-glucose
MOA: Increases Blood sugar
IND: Hypoglycemia
Con: Unconsciousness, unable to swallow, too altered to follow instructions
Adverse: Maybe asperated by pt without gag reflex, Nausea
Dose: Adult 15-45 grams orally placed between cheek and gum
Pedi 7.5 grams P.O. between cheek and gum
Specials: Watch for aspiration, Maintain airway, onset about 10 min, administer small amounts at a time. Pregnancy class C
D 50
dextrose 50% Carbohydrate
MOA: elevate BGL; glucose for cellular metabolism.
IND: Hypoglycemia.
CON: intracranial hemorrhage, Delirium Tremins (DT’s).
Adverse: Venous irritation, hyperglycemic.
Dose/Route: 10g-25g SIVP, IO
Pedi—>2y/o – 2ml/kg 25% 1:1 with sterile water <2y/o – 2-4ml/kg
SPCL: Class C pregnancy, Necrosis of the tissue if infiltrated.
