Venous disorders Flashcards
1
Q
Superficial Thrombophlebitis- Eti
A
- Partial or complete obstruction of superficial vein- due to local irritation/ injury, pregnancy, abd. CA, occult DVT
- Virchows triad- stasis, hypercoaguable, vessel damage
2
Q
Superficial Thrombophlebitis- Sx
A
- Dull vein pain
- Induration and erythema- linear along vein
- Cord-like vein
- Local to including saphenous vein
3
Q
Superficial Thrombophlebitis- Dx
A
- CBC- fever chills
- Culture if sepsis
- Underlying etiologies
4
Q
Superficial Thrombophlebitis- Tx
A
- Rest, heat, elevation
- Removal of saphenous vein
- Coverage for staph
5
Q
Chronic venous insufficiency- Eti
A
- Postphlebetic syndrome
- Chronic swollen limb- stasis dermatitis, subq tissue induration, ulceration
- after phlebitis- incompetent veins
6
Q
Chronic venous insufficiency- Sx
A
- Capillary leakage of fluid & RBCs
- Hg causes imflammatory rxn, fibrosis- brown pigmentation
- Itchy, scale rash at medial malleolus
- Pitting to induration edema
- Hypoxia -> ulcers
7
Q
Chronic venous insufficiency- Dx
A
- Venogram = gold standard
- Clinical picture highly diagnostic
8
Q
Chronic venous insufficiency- Tx
A
- Difficult- tx stasis- ulna boots, excision, grafts
- Dematitis- burrows solution, topical steroids, zinc
- stockings, elevation
9
Q
DVT- Eti
A
- 1 million pts/year
- DV lower legs, pelvis
- 80% in calf
Risk: immobilization, illness, estrogen, obseity, CA, Hx
10
Q
DVT- Sx
A
- Asymptomatic
- Dull to frank pain in calf, leg, buttock
- edema
- Distention of superficial collaterals
- Fever, tachy, cyanosis, SOB
11
Q
DVT- Dx
A
- Fibrin D-dimer
- Venography = gold standard-invasive
- Duplex doppler US
12
Q
DVT- Tx
A
- Prevention
- Stockings, elevation
- Anticoag.
13
Q
Varicose veins- ETi
A
- Inherited venous valve incompetence
- Poor return of blood- stasis and pooling
- Pressure-> stretching and vein distention
- 15% adults
- women who have been pregnant
- standing and lifting
14
Q
Varicose veins-Sx
A
- Fatigue, dull aching pain-> standing
- asymptomatic
- visible, dilated veins
- Trendelenberg test
15
Q
Trendelengerg test-
A
- superficial saphenous femoral valve versus perforator incompetence
- Compress, stand
- Superficial: varicosities flat
- Perforator- varicosities apparent within 35 sec
