Indications for specialist vasodilators
ACS, severe hypertension, anaesthesia, critical care, unstable, monitoring and IV route
Specialist vasodilators
PDEI, Mg, alpha 2 agonists, nitroptusside, dobutamine, hydralazine
Vasoconstrictors
Adrenaline, noradrenaline, dopamine. Synthetic - phenylephrine, non-catecholamine - vasopressin, angiotensin. Vasopressin, angiotensin ( non-catecholamine)
Mechanism of action of adrenaline
alpha and beta agonist
How does it affect beta and alpha receptors
B1 increased HR, increased CO, SV. B2 bronchodilation. Vasoconstriciton due to alpha receptors.
When is NA used?
ITU to rescue BP, in spectic shock. Potent effect on alpha1 to vasoconstrict, significant side effect profile. Used in cardiac arrest and anaesthesia aswell
Mechanism of action of NA?
Increase calcium
MOA of GTN, nitrates
Produces NO which increases cGMP and reduces calcium. Causes venous dilatation - decreased venous return (preload), decreased myocardial oxygen demand, increased coronary blood flow so better pump function
