How is vaginismus treated?
Avoid triggers - eg ongoing pain sex
Pelvic relaxation techniques
Sensate focus
Graduated vaginal dilators
What is lichen sclerosus?
This is a familial autoimmune disorder primarily affecting the vulva causing dyspareunia
What is the pathophysiology of LS?
Dermis becomes acellular and thickens due to deposition of collagen > contraction of collagen and distortion
How does LS present?
Pruritis early
Leukoplakia
Has is LS treated?
Avoid irritants
Topical steroids
DMARDs
Surgery for introital stenosis
What is lichen planus?
Autoimmune disorder similar to LS but affecting the vagina
How does lichen planus present?
Vulvar pain and ulceration
Discharge
How is lichen planus treated?
Oral or systemic steroids
Methotrexate or azathioprine
What is vulvodynia?
Chronic vulvar pain disorder of unknown aetiology
What causes vulvodynia?
Unknown, potentially triggers include chronic candidiasis, trauma during intercourse due to inadequate lubrication, sexual abuse and vaginal delivery
