Most common cause of gastroenteritis in USA?
norovirus
Norovirus, spread?
fecal-oral, need very few particles for infection => leads to outbreaks (schools, cruise ships, day care facilities)
Norovirus most common spread way?
human to human, but can spread via ingestion of contaminated food, eg shellfish, salad, fruit.
Viral gastroenteritis. Table. norovirus for what ages?
among all ages
Viral gastroenteritis. Table. rotavirsu for what ages?
common in unvaccinated age =<2 y.o.
Viral gastroenteritis. Table. CP? 3
emesis and/or watery diarrhea
abdominal pain
+/- fever
Viral gastroenteritis. Table. Dx?
CLINICAL
Viral gastroenteritis. Table. Dx - when consider that it is NOT viral?
bacterial/parasitic: grossly bloody stools, persistent fever and exposures (eg international travel, farm animals)
Norovirus, Onset?
1-2 days after exposure
Norovirus (nu cia turbut bus ir rotavirusui) = diarrhea is noninflammatory without blood or mucus in stool.
Fever, malaise, headache is common.

Viral gastroenteritis. Table. Tx??
FLUID REPLETION:
- oral for mild dehydrations (normal vital signs, dry mucous membranes)
- IV for severe dehydration (weak/rapid pulse, marked oliguria, incr. capilary refill time)
Viral gastroenteritis. Table. Tx - what about food?
Regular diet as tolerated (limit fats and simple sugars)
When need to do confirmatory testing (eg norovirus in the stool)?
atypical cases, eg symptoms > 7 days.
Fecal leukocytes not seen.
Viral gastroenteritis. Tx = supportive

Viral gastroenteritis. When symptoms resolve?
2-3 days.
Norovirus - how to kill virus?
NOT killed by alcohol based sanitizer OR standard cleaning solutions.
Prevention includes: diligent hand hygiene using soap and water AND cleansing of contaminated surfaces with bleach.
In UW case: norovirus - buvo in teenager;
rotavirus explanation = in unvaccinated < 2 y/o.!!!

Rotovirus if occurs in teenagers/adults?
mild infection or asymptomatic.
