UTIs Flashcards
what divides upper/lower UTIs?
ureter opening
most common UTI organism
E. coli
common symptoms of UTIs
dysuria frequency urgency odor cloudy urine suprapubic tnederness usually no discharge
risk factors for UTIs
neurogenic bladder
DM
urinary instrumentation
structural abnormalities (cyctocele)
who should be cultured no matter what
pregnant pts
suspicion of abx resistance
pt has multiple drug sensitivities
underlying complicating medical condtions
complicated cystitis
recurrent in female males urethral malformations turbulent urine flow neurogenic bladder nephorlithiasis immunocompromised renal disease prego DM catherization upper tract infections
asymptomatic bacteriuria
should not be Tx unless:
pregnant, DM (controversial), urinary outflow obstruction, anticipated catherization,
what organisms will not culutre
mycobacterium tuberculosis adenovirus polymavirus CMB anaerobes fungal interstitial cystitis (condition, not organism, not understood)
Tx UTIs
Trimethoprim/sulfmethoxazole
flouroquinolones
beta-lactams
aminoglycosides
flouroquinolones ADR
ruptures achilles, greater incidence when on concurrent steroids
beta-lactams
penicillins
cephaloporins
monobactams
carbapenems
aminoglycosides- ADR
ototoxic and nephrotoxic
