- E coli from nearby flora have colonized the perineum with the use of type 1 pili
- a few E coli gain access to the bladder via mechanical disruption (eg catheters)
3 bladder has expelled the E coli with type 1 pili only. those E coli have P pili will remain in the bladder and cause UTI with inflammation and hemorrhage - in some cases, E coli can ascent the ureter to cause pyelonephritis (kidney infection)
