Indications for Biopsy
A
- >5cm lesion
- deep to fascia
- overlying bone or NV structures
- (not a lipoma)

Principles of Open Biopsy
A
- Extensile incision – longitudinal in extremities
- Avoid developing planes
- Use involved compartment
- Do not expose neurovascular structures
- Meticulous hemostasis – pack***
- Release tourniquet prior to wound closure
- If using drain, bring out in line with incision
- For bone lesions, ok to biopsy soft tissue mass

4 Surgical Margins
A
- Intralesional – plane of dissection enters tumor
- Marginal – plane of dissection through reactive zone at edge of tumor
- Wide – tumor removed with cuff of normal tissue (2cm)
- Radical – entire compartment tumor occupies removed
