TOS & Neuro Tension Flashcards
Thoracic Outlet Syndrome
The compression of the neuromuscular bundle
Neurovascular Bundle
Brachial plexus
Subclavian vein
Subclavian artery
Brachial Plexus
C5-T1 B/w anterior scalene and middle scalene Under the clavicle Over 1st rib Under the Coro oil process posterior to pec minor
Possible causes of TOS
Cervical Rib
↓interscalene triangle (tight scalene)
↓costoclavicular space (costoclavicular syndrome)
↓costocorocoid space (pec minor syndrome)
↓space caused by shoulder droop
Callus formation from clavicles fracture
Adson’s Maneuver
Interscalene tightness (ant-middle)
- Radial pulse
- EX shoulder and Downward Traction
- px - Take a DEEP breath in and hold
- Neck EX and ipsilateral Rotation
Positive
- Diminished of pulse
- Reproduction of S/S
Travelle’s Variation
Halstead Maneuver
Mid scalene tightness
- Radial pulse
- EX shoulder and Downward Traction
- px - Take a DEEP breath in and hold
- Neck EX and CONTRALATERALLY Rotation
Positive
- Diminished of pulse
- Reproduction of S/S
Costoclavecular Syndrome Test
Military Stance
B/W Rib 1 and Clavicle
- Radial pulse
- Px - Shoulder Down and Back (Military Stance!!)
- px - Take a DEEP breath in and hold
Positive
- Diminished of pulse
- Reproduction of S/S
Wright’s Hyperabduction Test
Pec Minor / Axillary / Costoclavicular?
- Radial pulse
- px - Take a DEEP breath in and hold
- Passively ABduct the arm (no shoulder elevation)
* hold px’s arm like a guitar
Positive
- Diminished of pulse
- Reproduction of S/S
Roos Test
Elevated Arm Street Test
TOS, Neuro S/S , Brachial Plexus
- Px seated or standing
- No the arms at 90 degree of shoulder AB and ER
- Px actively bring elbows to 90 degree FL
- グーパーグーパー 3 minutes
Positive
- Inability to maintain upper limb position
- Ischemic pain
- Numbness and parasthesia
Allen’s test
TOS, Brachial Plexus (mid scalene)
- Px shoulder AB 90 degree with elbow FL 90 degree
- Radial pulse
- Px CONTRALATERALLY Rotaion
Positive
Radial pulse weakens or disappears
Upper Limb Neural Tension Maneuvers
Median Nerve / ULNT1
- Px Supine
- Shoulder girdle Depression & slightly AB shoulder
- Elbow EX with slight ER of shoulder
- Supination of forearm
- Wrist, finger and thumb ET
- Full AB
ULNT 2
Median, Musculocutaneous, Axillary Nerve
- Supine
- Shoulder girdle Depression and slight AB
- Elbow EX with FULL ER of shoulder
- Supination of the forearm
- Wrist, finger and thumb EX
- Full AB
ULNT 3
Radial Nerve
- Supine
- Shoulder girdle Depression and slight AB
- Elbow EX with IR of shoulder
- Pronation of the forearm
- Wrist, finger and thumb FL with wrist Ulber deviation
- CONTRALATERALLY Lateral FL of c-spine
ULNT 4
Ulnar Nerve
- Supine
- Wrist EX and Forearm Supination
- Elbow FL
- Shoulder girdle Depression
- = above position and add ER and AB
- Contralaterally Lateral FL c-spine
LLNT Maneuver
Sciatic Nerve
- Supine
- SLR position
- Elbow EX with FULL ER of shoulder
- Ankle Dorsiflexion
- Hip AD
- Hip IR
- Cervical FL
- Eversion to Dorsiflexion - more tension to Tibial Tract
- Inversion to Dorsiflexion - Surat Tract
- Plantarflextion with Inversion - Common Peroneal Tract
