Varus Stress Test
Joint stability (ligament) test
- Lateral collateral Ligament
- Posterolateral Capsule
- Occasionally Biceps femoris & IT band
Positive
- Pain
- Increase joint opening
Knee extended- pasterolateral capsule, Lateral collateral lig, biceps
Knee slightly flexed- Lateral collateral lig, It band
Apley’s Distraction test
BL collateral Ligament
Px- Prone
Th- 1. Distract - Pull up (Lift up) grab above malleoli
2. Rotate
Positive - Pain
Anterior Drawer Test
Anterior Cruciates Ligament
** Before this Test, Do Sag Sign!!!
Px- Hook-lying
Th- Sit on Px’s foot and Draw tibia forward
Positive
- Pain
- Increase movement
Sag Sign (gravity draw test)
Posterior Cruciate Ligament (PCL)
Px- Supine with hip and knee flexed
Th- See if the tibial Tuberosity visible and sagged(BL)
*Rule out to the PCL prior to testing ALC
Posterior drawer test
The integrity of the PCL
Px- Supine
Th- sit on the foot and move backward
Positive
- Pain
- Increase movement
Lachlan’s test
Tests the integrity of ACL
Px- Supine
Th- Femur stabilizing with one hand / other move proximal tibia forward
Positive
- Pain
- Increase movement
- Soft mushy end feel
Meniscus attachment site
Medial meniscus
- att to the tibial plateau via small coronary lig
- att to the medial collateral lig
Lateral meniscus
- att to the edge of the tibial plateau via coronary lig
- Popliteus but NOT to the lateral collateral lig
McMurray’s meniscus test
Posterior horns of meniscus
Px- Supine
Th-
1. grasp px’s heel and around knee
2. Fully Flex the knee - Rotate tibia IR and ER
3. Extend Knee (almost full) Tibia Externally extend - Valgus
Tibia internally rotate- Varus
Tibia ER- Valgus
Tibia IR- Varus
Heel IN/Push IN - Medial meniscus
Heel OUT/Push OUT - Lateral meniscus
Positive
- Pain
- Audible click
Apley’s compression test
Miniscus Test
Px - Prone and Flexed at knee
Th - Push down and IR/ER
Positive
- Pain
- Click
Bounce Home Test
Meniscus test
Px- Supine
Th- Flex the px’s knee up and possibly extend (fully Let Go)
Positive
- Knee stops short
- Rubbery resistance
Patella Femoral grinding (Clarke’s sign)
Chondromalacia patella (softness of the articulate cartilage)
Positive
- Pain in large% with NO S/S
- Palpable crepitus
Patella Femoral Compression Test
Patellofemoral Pain Syndrome
Quality of the articulate cartilage b/w the posterior aspect of patella and femoral condyles
Px- high seated and knee extended
Th- push the patella posteriorly against the Femoral condyle
- Repeat at 30, 60,90 degree of FL
Positive - Pain
Patellar Apprehension Sign/ Test
Patella dislocation/subluxations
Px- supine and knee FL 30 degree
Th- Push patellar Laterally (slowly and carefully)
Positive
- Px Apprehension
- Cotracting the quads to patella back in alignment
