Infection
Pathogenesis of TMD(things that may lead to the cause)
Inflammation of MoM/TMJ secondary to parafunctional habit
Trauma
Stress
Psychogenic
E/O signs of TMD
Joints- clicking/crepitus
Facial assymetry
I/O signs of TMD
Signs of parafunctional habit: cheek biting, linea alba, tongue scalloping, occlusal nctsl
Radiographs to be taken if there is suspicion of pathology in TMD
OPT

Arthrogram
Common clinical features in TMD
Trismus
Clicking/crepitus
Muscle/ear pain upon wakening
Headaches
Female
Age: 18-30
Reversible treatment of TMD
Pt education: counselling, electromyographic recording, jaw excercises
Medication: NSAIDs, tricyclic antidepressants, botox
Physical therapy: relaxation, ultrasound therapy, TENS, hypnotherapy
Splints: bite raising appliances, anterior repositioning splint
Counselling tx for tmd
Reassurance
Soft diet
Support mouth when yawning
Stop nail biting/grinding
Cut food into small pieces
Example of bite raising appliances
Anterior repositioning splint
Wenvac splint
Michigan splint
Lucia jig
How do bite raising applications work
Stabilize occlusion and improve function of masticatory muscles thereby decreasing abnormal activity
Protect from grinding
Treatment for irreversible TMD
TMJ surgery
Why does joint clicking occur in TMD
Lack of coordinated movement between condyle and articulator disc
What is most common cause of TMJ clicking
Anterior disc displacement with reduction
Signs/symptoms of anterior disc replacement with reduction
Jaw tightness/locking
Mandibular deviation
If left untreated, what may anterior disc displacement with reduction progress to
Osteoarthritis
