Iodine 131

A
  • require special precautions
  • need to teach patient what it will be like before and how they need to prepare
  • Very Toxic! Once taken, considered “hot” radioactive for 5-7 days
  • purpose: shrink thyroid by completely destroying the thyroid cells
  • can produce hypothyroidism because the thyroid dies and no functions are left
  • have to switch treatment from hyperthyroidism to hypothyroidism

Iodine 131 precautions

A
  • avoid other humans and animals for 5-7 days
  • use disposable dishes
  • keep all clothing and trash in room til radiation precautions have passed
  • sometimes is used to visualize the thyroid in a diagnosis ( amount is such a small dose don’t have to worry about exposure)

Lugols solution

A
  • decrease blood flow to thyroid
  • reduce the size of the thyroid while decreasing the production of T3 and T4
  • short term solution given in preparation for thyroidectomy

Saturated solution of potassium (SSKI)

A
  • decrease blood flow to thyroid
  • reduce the size of the thyroid while decreasing the production of T3 and T4
  • can be an emergency treatment for hyperthyroidism (so-called thyroid storm), as high amounts of iodide temporarily suppress secretion of thyroxine from the thyroid gland

Levothyroxine

A
  • Drug of choice for replacement therapy for treatment of hypothyroidism
  • synthetic T3 and T4 , mimics thyroid hormones
  • give in the early morning, so that it’s on a empty stomach and doesn’t mess with them being able to sleep later
  • teach to take HR
  • get labs regularly on T3&T4

Propylthlovracil PTU

A
  • given when needing to drop temp, BP, HR
  • IV -> BINDS to T3 and T4 and stops it from stimulating the sympathetic nervous system
  • a thyroid storm can happen shortly after surgery or during surgery-> use this to block the T3 and T4
  • can be used in combination to treat symptoms of thyroid storm
    • beta adrenergis blocker (decrease HR and BP)
