Hypo in areas of the world where iodide is sufficient
Explain the patho of hashimoto thyroiditis?
Autoantibodies against thyroglobulin and TPO.
2 classic histo features diagnostic of hashimoto?
Lymphocyte infiltrate with germinal centers and atrophic follicles with eosinophilic change called hurthle cells
Big picture, what is going on with hashitoxicosis?
Immune mediated assault which leads to hyperactivity which exhausts the follicular cells.
What are the 3 mechanism of attack they think is going on with hashimoto?
Cd8, macros, and autoantibody.
What does a typical thyroid gland look like with hashimoto?
Diffusely enlarged.
2 things to remember about granulomatous thyroiditis?
Most common cause of a painful thyroid
They think its origin is found in viral infections
2 things to remember about riedels thyroiditis?
Fibrosis and plasma cells producing IG4
What are 2 other diseases she wants us to know that are IG4 related?
Idiopathic retroperitoneal fibrosis and autoimmune pancreatitis
3 things to remember about subacute lymphocytic thyroiditis?
Painless, postpartum thyroiditis, and autoantibodies are against TPO
What is the thyroid state of subacute and granulomatous?
Can be hypo or hyper
