Thursday - Pituitary Path - Krafts Flashcards
Most Pathology of pituitary is what lobe?
Anterior
mnemonic for basophils in anterior pituitary
B Flat FSH LH ACTH TSH
Most common cause of Hyperpituitarism
other causes:
Pituitary Adenoma
destruction of end organs
hypotalmus disorders
hyperplasia of anterior lobe
carcinoma of anterior lobe
Findings in pituitary adenoma
no endocrine things for a while
first, visual defects (bitemporal hemi-anopsia, later fields gone) - increased ICP
hypo pit!
Complications of pituitary adenoma
can bleed suddenly
can be “invasive”
usually an incidental finding
What does a pituitary adenoma look like
highly variable, not a single description.
pleomorphic or uniform

amenorrhea, galactorrhea
dopamine agonist
Groth hormone adenoma before puberty called?
after puberty?

Acromegaly
- mostly face and hands
Other complications of growth hormone adenoma
diabetes
hypertension
artritis
gastric carcinoma
ACTH adenoma can cause
Cushing syndrome (also disease)
Nelson syndrome (if you remove the adrenals, which you won’t)
Causes of Hypopituitarism
Pituitary destruction
- surgery, radiation or adenoma that’s too big
ischemic necrosis
- Sheehan syndrome from hemorrhage in pregnancy
Empty Sella Syndrome
Pituitary apoplexy
- sudden infarct, can be deadly
Clinical presentation of of hypo pit
decreased everything
- GH –> weakness
- FSH/LH –> no libido
- PL no lactation
- TSH - hypothyroid
- ACTH - adreanal isufficiency
2 types of Diabetes insipidus
due to?
Blood is hypo or hypertonic?
Central, nephrogenic
decreased ADH (or don’t respond to it)
- trauma, tumor, alcohol
Hyper
SIADH due to:
Blood is hypo or hypertonic?
what cancer causes it?
increased ADH
hypotonic
small cell lung cancer
