pylocytic astrocytoma

most often in children - low proliferative potential (e.x. meningiomas). histo: very pink rosenthal fibers with GFAP (glial fibrillary acid proteins.) relatively favorable outcome

you should see it in normal astrocytes, but you have way more astrocytes in astrocytomas (go figure) so you see a lot of it.

Grade 4 primary brain tumor

A

4 - Glioblastoma, primary or secondary (secondary meaning derived from grades 1,2,or3.)

cytologically malignant, mitotically active. PSEUDOPALISADING NECROSIS means the tumor cells are trying to get away from the necrosis. often pathologists will describe how many mitotically active cells they see on a slide. usually fatal.

survival depends. GBM ~1year. state of the art Tx may be 5+ years.

(primary glioblastoma is the most common primary brain tumor in adults. sometimes can see a “butterfly” glioma, where it crosses the midline of the brain.)

Ependymoma

A

Usually in children, usually near fourth ventricle

adult variant usually seen in spinal cord, or usually grows toward the spinal cord

dont see blood supply on gross disection

histo: see a “rosette” or “canal” or a circle of cells that are around fibrillary proteins (central lumen)
also: vascular pseudorosettes. cells are seen to kind of back off of blood vessels

medulloblastoma

pearls

A

WHO grade 4

usually in children

small dark, elongated, anaplastic cells

homer-wright rosettes. looks like little stars.

Sx: headache, morning vomiting, problems with motion

more prominent in males

rare, in immunosuppressed

easily detectable in CSF

agressive, respond poorly to chemo

large, B cell lymphoma

third most common brain tumor

frequently located at CN VII at cerebellopontine angle (get hearing loss)
