What TOS test should be done before all others?
Allen’s
Name the test: Locate radial pulse, patient turns head toward involved side, deep breath and hold, loss of pulse - positive. If negative, patient turns head to uninvolved side, check again. Positive suggests interscalene compression
Adson’s Test
Name the test: Patient is seated both hands at sides, doctor locates radial pulse on both forearms and passively abducts them to 180 degrees (separately) - notes the degree at which radial pulse disappears on affected side - as compared to unaffected side. This test stretches the pec minor and is significant for neurovascular compromise of the axillary artery
Wright’s test
Name that test: Patient is standing and flexes trunk forward so the doctor can get a view of their back. Checking to see if there is any inclination from one side to the other. A trunk angle of greater than 7 degrees is an indication of a structural curve of at least 20 degrees.
Adam’s test
Name that test: Patient is standing and raises both arms overhead, then bends laterally to both sides. Pain on concave side is intercostal neuritis, while pain on convex side is intercostal myofascitis (most likely)
