Normal cervical ranges of motion:
F: 50 degrees
E: 60
LF: 45
Rot: 80
Cervical ROM testing is performed - both passive and resisted ROM.
O’Donaghue’s Maneuver
Pain w/passive = ligament sprain, pain with resisted = Muscle strain
Patient supine - head is passively flexed while sternum is stabilized
Soto-Hall
Any muscle or ligament injury to posterior aspect of cervical spine would produce pain
Patient seated - shoulder is depressed while head is laterally flexed away
Shoulder depression test
Patient is seated, doctor behind, gently grasps both sides of the head and applies upward axial distraction
Cervical Distraction
Increased pain: Muscle spasm
Relief of pain: indicates foraminal encroachment or facet capsulitis
Patient presents with neck and arm discomfort. You ask them to place their hand on top of their head.
Bakody’s sign
Radicular pain decreased: nerve root compromise
Patient is seated and neck is passively flexed to their chest
Lhermitte’s Sign
Sharp pain down spine and upper/lower extremity = cord myelopathy
Patient is Supine - head is passively flexed. Positive if both knees flex
Brudzinski’s sign
Indicative of meningitis
Normal ROM for thoracic spine:
F: 20-45 degrees
E: 25-35
LF: 20-40
R: 35-50
