A
Diuretic
- inhibits re absorption of NA+ and Cl- from distal renal tubule
- increase Ca reabsorption

A
- HF stage A (chlorthailadone)
-high coronary disease risk (chlorthalidone)
-diabetes (chlorthalidone)
stroke (peirdoril + indapamide)

adverse effect
A
- Hypokalemia and other electrolyte imbalances
- Negative effect on glucose (intolerance; insignificant +3-5mg/dL) and lipids
- Sexual dysfunction

comments
A
- Avoid in pts with sulfonamide allergy, except ethacrynic acid
- Chlorthalidone= 2X more potent than HCTZ
- Thiazide diuretics are more effective as anti-HTNs than loop diuretics
- Caution in pts with gout
- Ineffective when CrCl <30ml/min

- Electrolytes (hypo-K, Na & hyper-Ca)
- Uric acid
- BUN, SCr
- I & O fluids
- 7-14 days after initiation
