Preinteraction phase
- the RN obtains info about pt from the chart, family or other members of the healthcare team
- RN examines own feelings, fears, anxieties about working with this particular patient
Orientation (introductory) phase
- RN creates environment of trust and support
- RN meets pt, develops rapport
- gathers assessment data
- identifies pt strengths and limitations
- formulates diagnoses
- sets mutually agreeable goals
- develops plan of action
Working phase
- works to maintain trust and rapport
- promote pt insight and perception of reality

Boundaries in the nurse/client relationship
- a boundary indicates a border or limit. It determines the extent of acceptable limits.
- concerns regarding professional boundaries: self-disclosure, gift giving, touch, friendship, romantic association
Therapeutic communication
- caregivers verbal and non-verbal techniques that focus on the care receivers needs and advance the promotion of healing and change
- encourages exploration of feelings and fosters understanding of behavioral motivation

- to listen actively is to be attentive to what the client is saying, both verbally and non verbally

- present in a manner that discourages defensiveness from pt.
