Right Drug
check the medicine order with the label 3x know the reason why drug is being given is the drug appropriate know the generic and the trade name check expiration and possible allergy
Right Dose
check if the dose is appropriate for the patients age and size
recheck mathematical calculations
Note difference between volume and mass
Right Time
within 1/2 hour of prescribed time
Military time used
consider drug-drug and drug-food interactions
do NOT abbreviate the drug order
PRN medications
making PRN decision- nurse’s discretion, how much to give and when to give
Requires- reason for drug use and assessment of patient’s needs
Right Route
IV =/ PO
drug dosage varies with drug route
Right Patient
verify the identity of the patient, ask for the patient’s name, and young children may or may not identify themselves
Right Documentation
record and sign necessary forms and documents
medication administration record lists the medications a patient receives includes dose, time, route
ADVERSE DRUG EVENT
all problems caused by a medication or failure to administer medication
Monitoring- evaluation of the patient to determine effectiveness and watch for harmful effects
two types of adverse drug events
1. adverse drug reaction
2. medication error

unexpected, undesired or excessive response of medication occurring at therapeutic levels
