DIAGNOSIS
3 Steps process in writing a diagnosis

- Identify the cause or risk factors
- supply evidence to support the claim
PLANNING
Setting priorities, formulation nursing interventions and setting goals and outcome criteria
IMPLEMENTATION
Measures to reach goals/outcomes Medication administration promoting compliance monitoring labs minimizing side effects observing for complications/side effects
EVALUATION
Evaluating results patients response to medication goals are met/not met changing goals side effects patient compliance
Client Teaching
Nurse always has the responsibility to teach the client/patient
