the eyeball lady that didnt use a mic Flashcards
number of bones in the orbit
7
orbital floor
three bones, palatine bone, maxillary and zygomatic
what is the weakest part of the orbit?
orbital floor, weakest closest to infraorbital nerve
medial wall
sphenoid, maxillary, ethmoid, and lacrimal
thinest wall of orbit?
medial wall
ethmoid foramen
passage of nerves and vasculature into nose
orbital roof
two bones, frontal bone and lesser sphenoid wing
lateral wall
thickest bone, greater sphenoid, and zygomatic
muscles
6 muscle, but four primary
looking up?
superior rectus
looking down
inferior rectus
torsion
superior and inferior oblique
look medial
medial rectus
look lateral
lateral rectus
innervation
cn3, 4, 2, and 6
cn3 innervation
superior rectius and inferior rectus
cn4 innervation
superior blique
cn6 innervation
lateral rectus
cn2 innervation
the eye
sensory?
cn5, v1 = nasocilliary, frontal nerve, and lacrimal
v1 = maxillary - infraorbital nerve (cheek numbness), zygomatic nerve
oculocardiac reflex
increased pressure on eye leads to bardycardia, arythmias
efferent branch is connect to vagus, hence influence on heart. atropine, glycopyrrolate can help is release doesnt help.
orbital fractures
floor and medial wall - most common (thinnest)
the floor leads to cheek numbness due v2 compression
enphthalmos - shrunken eye, incarceratedmuscle, 50% wll fracture = needs surgucal repair
eye hurts for 2 days and is swollen
cellulitis!
can open eye - preseptal cellulitis
cant open eye - orbital cellulits
infection in sinus
can travel across into orbit leading to medial cellulitis
orbital cellulitis
presents with pain, erthematous lid, swelling
eye is appears red, muscle constriction, increased IOP, proptosis
can cause vision loss, pupillary issues
treat with broad spectrum antibiotics, use contrat and find abcess - treat with surgery
cavernous sinus thrombosis
bugs can travel through facial veins into cavernous sinus, leads to lots of nerve pulseys. major complication of orbital cellultis. altered meltal status
superior and inferior opthalimic veins are the routes of travel.
canthes
corner of eye, medial and lateral
eye lid blood supply
marginal arcades and periferal arcades
preseptal cellulits
can open eye
chalazion
backing up in meibomian glads wthin tarsus casing swelling. not infected!
use warm compression for treatment
eyelid trauma
dont want to tear the levator
avulsion
full thikness slice! looks like bacon hanging off the orbit. lots of surgery
see fat?
check levator
anterior structors of eyelid
skin and orbicularis
posterior
tarsus and conjunctiva
dacryocysilitis
infection of nasal lacrimal sac due to duct obstruction
need bypass surgery
double vision onset of pain, eye drop (pointed downward)
the lacrimal gland, has a little tumor in the gland pushing on the eye.
this looks like cn3 paulsy, so maybe think uncus herniation too?
