Tetanus (What is it?)
Exotoxin produced by Clostridium tetani, gram positive anaerobic rod
Pathogenesis of Tetanus?
Spores enter through wound (ie. puncture) and regenerate under anaerobic conditions
Tetanus acts on which nervous system?
Central and Peripheral
Incubation period of tetanus?
3-21 days
Sx of Tetanus? (Think: TOP to BOTTOM)
- Lockjaw/Trismus
- Neck stiffness
- Difficulty swallowing
- Muscular rigidity
- Severe muscle spams
How do neonatals contract tetanus?
Un-vaccinated mothers through an infected umbilical stump
What are the 7 complications of tetanus?
- Laryngospasms
- Fractures
- HTN
- Nosocomial Infections
- PE
- Aspiration PNA
- Death
Are there specific laboratory findings for a patient with tetanus?
No
What type of patients are at risk to contract tetanus?
- Drug users (ie. inject-able drugs)
- Multiple tattoo
- Body piercings
What is the typical management of tetanus? Describe.
Supportive care (ie. Hospitalization, airway management, anti-spasmodics)
What two medications can be given to treat tetanus?
- Tetanus Toxoid (Vaccination)
2. Tetanus Immune Globin (TIG - given IM)
