Rifampacin mechanism
Inhibit bacterial DNA dependent RNA polymerase preventing transciption of DNA to mRNA
Isoniazid mechansim
Inhibit mycolic acid synthesis
Pyrazinamide mechanism
Convert pyrazinamidase iinto pyrazinoic acid which inhibits fatty acid synthase I
Ethambutol mechanism
Inhibits arabinosyl tarnsferase which polymerises arabinose into arabinan
Rifampacin side effects
Orange secretions
Hepattis
Flu like symptoms
P450 inducer
Isoniazid side effects
Peripheral myopathy
Hepatitis
Agranulocytosis
P450 inhibitor
Pyrazinamide side effects
Hyperuricaemia –> gout
Arthralgia, myalgia
Hepatitis
Ethambutol side effects
Optic neuritis
TB treatment protocol - active
2 months RIPE then 4 months RI
TB treatment protocol - latent
3 months RI or 6 months isoniazid
TB treatment protocol - meningeal
Minimum 12 months treatment
Steroids
What is important with isoniazid?
Prevent peripheral neuropathy with pyridoxine (vit B6)
