Who is overall responsible for the safety of the entire command?
Commanding Officer or OIC
When is Tag Out training conducted?
Annually, when reporting on board.
When do we use a caution tag?
When providing temporary special instructions or to indicate that unusual action must be exercised to operate equipment.
When is a Tag Out audit conducted?
- Every two weeks per TUM and every week when in ESRA.
- Weekly on board USS HOWARD.
- MLOCS
In situations where the RA representative is no longer available to authorize clearance, how can the tags be cleared?
The authorizing officer may authorize clearing the tag by marking block 20b “RA Not Available”.
When a tag is missing during an audit, what action and procedure must be done with the missing tag?
Stop all affected work and take appropriate action to ensure continued protection of personnel and equipment until replacement tag is posted. Verify equipment is in correct alignment. Replacement tag for a missing tag will be assigned the same number as the original tag.
When is a danger tag used?
When prohibiting operation or removal of equipment that could jeopardize safety of personnel or endanger equipment, systems or components.
When is out of commission label or sticker used?
If the instrument error is large or inconsistent.
When is out of calibration label or sticker used?
Orange label, if the instrument error is small and consistent, the magnitude, sign, and units of the required correction should be marked on the label.
What types of systems, equipment, components require a double barrier protection?
- High temp 200*
- High pressure 1000 PSI
- All sea connected systems.
- All hull penetrations below the waterline.
- Fluids with flash point below 200*
- Oxygen, hazardous, toxic fume systems.
What if a two barrier protection is not feasible?
Requires CO permission for single barrier protection.
