Syncope Flashcards
what does TLOC stand for?
transient loss of consciousness
what is TLOC?
a state of loss of consciousness that only lasts a short duration
name four causes of TLOC
head trauma
syncope
epileptic seizures
TLOC mimics
what does the initial evaluation of a patient with TLOC involve?
careful history
full physical exam
orthostatic BP
12 lead ECG
what is syncope?
transient loss of consciousness due to cerebral hypoperfusion
what are the three categories of syncope?
reflex syncope
orthostatic hypotension
cardiac syncope
what is reflex syncope?
syncope in which neural reflexes modify heart rate and vascular tone, resulting in a fall in MAP = hypoperfusion
what are the three types of reflex syncope?
vasovagal syncope (VVS)
situational syncope
carotid sinus syncope (CSS)
what is the most common type of syncope?
vasovagal syncope
what triggers vasovagal syncope?
emotional or orthostatic stress
what is VVS associated with?
pallor
sweating
nausea
what can be done to avert VVS?
adopting horizontal gravity neutralising position
leg crossing
these both increase venous return
what is the main risk associated with VVS?
injury when falling
what is situational syncope?
faint during/immediately after a specific trigger i.e. cough, swallow etc.
how is situational syncope treated?
treat cause if possible
lie down if expecting faint
hydration
cardiac pacing
