Trapezius and Sternomastoid Muscle Innervation and Blood Supply
How to determine clinically if nerve is intact?
Accessory Nerve (CN XI)** Transverse Cervical A / V
Both are Deep to the Trapezius
To determine if the Accessory nerve is intact, have patient raise shoulders, if one droops it might be the accessory nerve
Rhomboid Major and Minor and Levator Scapulae Innervation
Dorsal Scapular Nerve
Rhomboids act to retract scapula and rotate it to depress the glenoid cavity
Levator Scapulae act to elevate the scapula
Clinically test the dorsal scapular nerve to see if it is intact
Latissimus Dorsi Innervation
Thoracodorsal A/N**
Extends, adducts, and medially rotates the upper extremities
Serratus Anterior Muscle Innervation
Long Thoracic Nerve
Lateral Thoracic Arterie
Serratus Anterior protracts the scapula and holds it against body wall. Facilitates rotation of the scapula.
Winged Scapula
Caused by injury to the long thoracic nerve. Serratus Anterior muscle is not intact and does not hold scapula to body.
Triangle of Auscultation
Formed by the three structures:
Trapezius
Latissimus dorsi
Medial border of the scapula
and Rhomboid Major
Deltoid Innervation
Innervated by Axillary Nerve on posterior deep surface
Clinical Function is abductor of upper extremity
Quadrilateral (Quadrangular) Space
Formed by Teres Major and Minor, and also the long head of the triceps and the humerous
The Axillary Nerve and posterior humoral circumflex artery (forming the neurovascular bundle) pass through this space
Clinically - when giving an IM injection try to miss the axillary nerve which runs through this space
Subacromial Bursa
Located deep to deltoid and acromion.
Infammation of bursa will cause pain
Painful Arc Syndrome
Calcification of the supraspinatus tendon leading to inflammation of the bursa causing pain on abduction of the upper extremity
Supraspinatus and Infraspinatus Muscle Innervation
Innervated by the Suprascapular A/N **
Supra - abducts upper extremity
Infra - rotates upper extremity
Rotator Cuff Muscles
Supraspinatus
Infraspinatus
Teres Minor
Subscapularis
Teres Minor Innervation
Axillary Nerve
Laterally rotates the upper extremity
Teres Major Innervation
Lower Subscapular Nerve
Transverse Scapular Ligament
Bridges over the suprascapular notch (in elderly this may turn into bone)
The Suprascapular nerve passes under the ligament, the suprascapular A / V pass above it
