What are the risk factors for psoriasis?
Genetic - 30% pts have first degree relative with it Psychological Stress Medications Infection HIV
What are precipitating agents for psoriasis?
Infection, trauma, stress, ETOH Systemic steroids Beta blockers Lithium Antimalarials Indomethacin
Psoriesis that occurs on the extensor surfaces - knees, elbows, pre sacral, nails
Palms of soles, thick scale of the arch of the foot and thenar and hypothenar palms
Chronic Plaque type psoriasis
Psoriasis that occurs in folds of skin - gluteal fold, axillae, glans of penis, Usually not scaly
Inverse Type psoriasis
Psoriasis that occurs after a streptococcal infection, usually in young adults or children. Sudden onset with small tear drop shaped scaly papules of trunk and proximal extremities
Guttate Type Psoriasis
Psoriasis that occurs with small pustules becoming generalized with fever - If localized involves the soles of hands and feet..
Pustular Type Psoriasis - Can be life threatening
If localized called Pustular Psoriasis of Barber
What is psoriatic Arthritis?
Inflammatory seronegative arthritis with variable course - asymmetric and involves fingers and toes Occurs in 1/3 pts with psoriasis
What disease can be defined by the 6 Ps? What are they?
Lichen Planus - chronic autoimmune inflammatory disease
Planar (flat topped) Purple Polygonal Pruritic Papules Plaques
Who is the common target of Lichen Planus?
Usually in premenopausal females (age 30 - 60)
Associated with Hep C
Located on the wrists, shins, mucous membranes
Defined by Wickham’s Stria - Lacy, reticular, white lines
Maybe pt says ‘its a rash thats been there for a year and has not gone away’
What are the common features of oral lichens plans?
It can occur by itself or along with classical lichen planus (except for drug induced LP);
Characterized by tender red patches in buccal mucosa (lining of inside of mouth) with a white surface that does not wipe away (as does thrush - fungal)
What is defined by a photosensitive pattern of erythema with butter fly rash across the nose and cheeks, along with erythmetous rash across sun exposed areas of upper chest and arms.
Acute Lupus Erythematosus
May become bullous and also may have non-specific features like digital infarcts (raynaud’s syndrome)
What is Raynaud’s syndrome
A common side effect of disease such as acute lupus erythematosus - characterized by reduced blood flow to distal digits of hands and feet - may cause death in the tissues..
What characterizes subacute cutaneous lupus?
Erythmetous and usually scaling rash of upper trunk and extremities, either psoriasis plaque like form or annular polycyclic form (ringlike, multiple circles)
The four types of chronic cutaneous lupus are?
Discoid - scarring lesions
Tumid - Erythamatous plaques in sun exposed areas
Lupus Panniculitis - destruction of adipose tissue
Verrucous Lupus - very thick hyperkeratotic discoid lupus
