Strokes Flashcards
Presentation of Stroke
weakness on one side of body
inability to speak
vision loss
vertigo/ falling
Presentation of Deep Vein Thrombosis
DVT
leg swelling
pain, warmth
Presentation of Pulmonary Embolism
PE
cough
chest pain
shortness of breath
hemoptysis
MoA of Aspirin
irreversibly inhibits cyclooxygenase, so cant make TXA2
Aspirin Use
prevent transient ischemic attacks and stroke
TXA2
causes platelet aggregation
Side Effect of Aspirin
bleeding
Clopidogrel
“Grel”
ADP Receptor blocker
stops ADP mediated platelet activation
Use of Grels
prevent TIA and strokes in people who cant take aspirin
Side Effects of Grels
bleeding
Abciximab
Glycoprotein 2b/3a inhibitor
blocks binding of fibrinogen with 2b/3a, preventing platelet activation
Use of Abciximab
coronary angioplasty to prevent clots
Dipyridamole and Inamirone
Phosphodiesterase Inhibitors
blocks breakdown of cAMP
cAMP prevents platelet aggregation
Alteplase
clot buster
given w/in 3 hours to decrease mortality
MoA of Alteplase
binds to fibrin bound plasminogen, converts plasminogen to plasmin
plasmin breaks clot
Heparin
catalyzes binding of antithrombin 3 to factors 2, 9, 10, 12; causing inactivation
Heparin Use
IV for rapid anticoagulation for VTE
Side Effects of Heparin
bleeding
heparin induced thrombocytopenia (5+ days)
Antidote for Heparin
protamine sulfate
Enoxaparin
low molecular weight heparin that binds to antithrombin 3 that negates factor 10
Warfarin
decreases synthesis of vit K dependent factors (2, 7, 9, 10)
prevents regen of Vit K
Warfarin Use
oral
long term anticoagulation
Antidote for Warfarin
Vitamin K
rapid reversal with frozen plasma
Dabigatran
directly inhibits thrombin
