Derived from the internal carotid artery
MCA or ACA
Supplies a large area of the frontal, parietal, and temporal lobes
occlusion causes dysfunction of face and upper extremity, language and speech
MCA
supplies the medial portion of the frontal and parietal lobes
occlusion causes dysfunction of the cortical area supplying primarily the lower extremity
ACA
derived from the basilar artery
PCA
supplies the occipital lobe
occlusion affects vision
PCA
L hemiparesis
visual-perceptual deficits
poor judgement, cognitive/behavioral issues
R stroke
R hemiparesis
language deficits
apraxia
processing delays, perseveration
L stroke
atherosclerotic plaques in first major branchings of large cerebral arteries. progressively narrows
thrombus
common risk factors for thrombus
HTN
DM
cardiac/vascular disease
Thrombus from outside brain (typically from plaque in carotid sinus, internal carotid, heart)
lodges in medium-sized vessels
immediate impact
symptoms vary if clot moves
Embolic CVA
abnormal bleeding in brain
more global problems than ischemic CVA - ischemic injury to area supplied by vessel - mechanical injury from blood, edema to distant neurons
Hemorrhagic CVA
Temporary interruption of brain blood flow, neuro symptoms last
Transient Ischemic attack
medically stable with functional disability and adequate endurance. cognition intact
rehab hospital
rehab unit of hospital or rehab center
more serious disability, less endurance, cognition intact
sub-acute care
skilled nursing facility
Need for long term 24 hour care because of cognition, comorbidities, lack of home support
Long-term care
