Skin appears
“no lesions, discolorations, texture is normal”
Begin exam by inspecting and saying:
“Shoulders, elbows, wrists, and fingers are symmetrical without atrophy, deformities, fasiculations (twitch) or tremors; there is no swelling or discoloration”
Palpate shoulder saying
“I am palpating the following areas: clavicles, acromioclavicular joint (AC joint), coracoid process and bicipital groove, as well as triceps and biceps”
Palpate elbow saying
“I am palpating epitrochlear lymph nodes, brachial pulse, lateral and medial epicondyles, olecranon process, radial and ulnar styloids, ANTECUBITAL fossa”
Palpate the wrist saying
” I am doing a general inspection of carpal bones, MCP jnts, PIP and DIP jnts, radial and ulnar styloids, RADIAL PULSE”
Ask pt if MCP PIP and DIP jnt are tender.
“There is no swelling or tenderness of PIP and DIP joints”
Palpate the hand sayingZ
“I am checking the nails for capillary refill.
Palpating metacarpophalangeal joints, proximal and distal interphalangeal jts. (Ask pt if tender) There is no swelling or tenderness of PIP or DIP joints.”
fasiculations mean
twitch
When testing ROM of upper extremeties, say
“Muscle tone and strength are normal and symmetrical. There is no rigidity or cog-wheeling”
cog-wheeling is
???
To test flexion and extension, say
“Put your hands in front, bring the all the way up above your head, Extention: swing arms back down and behind you.
To test strength resistance for abduction
Raise your arms bilaterally above your head, now hold your arms straight out to your sides at shoulder level and don’t let me push them down.”
To test for adduction and strength resistance
- swing your arms across your body
- hold your arms straight out to your sides at shoulder lever and don’t let me push them up.
To test external rotation
bring your hands behind your head.
Internal rotation: put your hands behind your back
ROM at elbow
Bring your fists to your shoulder and down.
Check flexion and extension of elbow and against resistance
Flexion: flex your elbow and don’t let me pull your arms down
Extension: extend your arms. Now flex the arms and push them against my resistance.
