What is the action?
Blocks norepinephrine at all alpha & beta receptors in the SNS
↓ BP & HR
↑ renal perfusion
↓ Renin
SNS blockers Alpha & beta drug names?
Carvedilol
Labetalol
What are the contraindications/cautions?
Allergy Heart block (beta cell blocked ↓ HR) Bradycardia (beta cell blocked ↓ HR) Lung disease/bronchospasm (block beta 2 receptors) Diabetics (alteration in glucose)
What are the drug-drug interactions (DDI)?
Antidiabetic agents (↑ the effectiveness of these drug) when given w/calcium channel blockers (electrical system problems within heart)
What are the indications (use for)?
HTN
What are the adverse effects?
CNS: Fatigue, HA, Dizziness, memory loss, insomnia, disorientation
CV: Arrhythmias & hypotension
Pulmonary: bronchospasm, cough, laryngospasm, pulmonary edema
GI effects: N & V, diarrhea
GU effects: Impotence, hypoglycemia, rash
What are the nursing implications?
Do not stop abruptly (can cause MI, stroke, or arrthymias)
Ineffective clearance r/t lack of bronchodilating effects
Hx: contraindication/cautions
CNS: cardiac, respiratory, abdominal, urinary, perfusion
Vitals, ECG
Labs: electrolytes, renal/hepatic function, glucose
Take apical pulse; hold if below 60
Monitor blood glucose
Lifestyle changes
change position slowly
Do not crush extended-release formulations
