Speeds
Pt seated
Forearm ext + sup, arm at side
Stabilise shoulder + wrist
Pt flexes arm to 120 (swing forward)
What does speeds test for
Bicep tendinopathy, impingement syndrome, labral lesion, bursitis, SLAP lesion
+Ve - Speeds
Pain
Empty can
Arm out horizontally thumb up
Pronate hand so thum is down - ‘empty can’
What does it test for - Empty can
Impingement or Injury/lesion to supraspinatous
+Ve - Empty can
Irritation + P- may be injury to muscle which produces IR (pec major, ant delt, lats)
Painful arc
Normal active abduction, open palms all the way up
What does it test for - Painful arc
Sub-acromion impingement
+Ve - Painful arc
60-120 local P to subacromial/rotator cuff
170-180 pathology to AC
Nears
Depress scapula
ER arm, bring to maximal flex
Repeat for IR
What does it test - Nears
ER- bicep long head impingement
IR- supraspinatous impingement, subacromial bursitis
+Ve - Nears
ER- P
IR- P
Crank
Pt seated
Abd + ER elbow at 90
Apply pressure into elbow
‘Brush hair’
Rotate humerus int/ext
What does it test for - Crank
Labral tears
+Ve - Crank
P + crepitus
If P is over AC test = -Ve
Obrien’s
Pt seated
Shoulder flexed to 90, arm add
Pt attempts to maintain position whilst you apply force on forearm
1. Arm IR, thumb down
2. Arm ER, thumb up
What does it test - Obrien’s
Labral tears, SLAP lesions, Ac pathology, bicep long head tendinopathy
+Ve - Obrien’s
IR- P- labral or SLAP lesion, AC pathology
ER- bicep long head tendinopathy
Clunk
Pt supine
One hand under GH, other supporting above elbow
Apply translation under GH, other hand ER humerus
What does it test for - Clunk
Labral lesion
+Ve - Clunk
Clunk or grinding sound- trapping of torn labrum between glenoid + humeral head
Kim
Pt seated
Back support ]stand on injured side
Abd to 90
Apply force through shoulder
Other hand on deltoid
Elevate their arm upwards, whilst apply downward force
What does it test for - Kim
Labral lesion
Kim’s +Ve
P + clicking
