Pt prone
Feet off couch
Squeeze calves
What does it test - Simmonds
Achilles tear/rupture
+Ve - simmonds
No plantar flexion
Talar tilt
Pt seated or supine
Place pillow under knee
Hand placement on lat + med malleoli
Full plantar flexion and invert foot - anterior talo fibular
Neutral - calcanofibular lig
Full Dorsi flex - posterior talo fibular
Neutral and eversion - deltoid lig
What does it test - Talar tilt
Inversion- lateral sprain (ant/post talofibular ligament)
Eversion- medial sprain (deltoid lig)
+Ve - Talar tilt
Inversion- lateral sprain/rupture= P + increased motion
Eversion- med sprain= P + increased motion
Ant/post draw
Pt supine
Bend leg, foot on couch
Fix top of foot
Support round back of Achilles, pull forward
Post- push shin
What does it test - Ant/post draw
Ant talofibular ligament instability
Post ankle ligament instability
+Ve - Ant/post draw
Increased ant movement
Increased post movement
Tinsels’s sign
Pt supine with hip of affected side ER + foot slightly everted
Tap over tarsal tunnel
What does it test - Tinsels’s sign
Tarsal tunnel syndrome
+Ve - Tinsels’s sign
Radiating P in foot + toes due to aggravation of tibial N
Homan’s sign
Pt supine with leg straight
Raise leg to 10 deg
Forcefully dorsiflex foot + squeeze calf with other hand
What does it test for - Homan’s sign
Thrombosis
+Ve - Homan’s sign
Deep P in calf
