Shock part 4 lecture Flashcards
Dopamine receptors get activated at lower concentrations as you increase dopamine it will activate beta 1 and alpha receptor
When you activate beta 1….cardiac output goes up…blood pressure=cardiac outputxtotal peripheral resistance
If you increase cardiac output your going to increase the blood pressure
ADRs of dopamine is __________and arrhythmias
tachycardia
Dopamine has a higher incidence of arrythmias and mortality
Epinephrine (Epi) has the strongest beta effect
Norepinephrine has the strongest alpha effect
Where are beta? heart
where is alpha? vascular smooth muscle cell
When it comes to norepinephrine they prefer beta 1 over beta 2
When it comes to anaphalaxis with bronchoconstriction which one do you need to use?
epinephrine because norpeinephrine targets beta 2
Norepinephrine strongest effect is through alpha 1 and it causes vasoconstriction
decrease renal blood flow
Barorecptor reflex=these receptors sense the mechanical stretch of the blood vessels and therefore sense and send signals to central nervous system…if the blood pressure is going down then it activates sympathetic autonomic nervous system.
If blood pressure is going up then it will activate parasympathetic autonomic nervous system so its like a monitoring station where it can sense stretch of the blood vessel due to the blood pressure and send signal back and forth btwn CNS and the rest of the body
Norepinephrine ADRs
Decrease in renal perfusion due to vasoconstriction
ischemic necrosis due to ________________
extravasation
Epinephrine acts on alpha and beta but strongest effect on beta
Beta 1–>major effects
inotropic effects (increase contracility)
chronotrophic effects (increase heart rate)
Low dose of epinephrine effects beta 2 receptor first so causes vasodilation in skeletal muscle
High dose of epinephrine so alpha 1 will predominate increase in TPR and BP which increases blood pressure
Epinephrine ADRs
Renal perfusion _________
Increase in blood _________ levels
decrease
increase blood glucose
Phenylephrine is alpha selective agent
pro is increase vascular tone (increase in TPR and increase BP
Cons: also decrease CO due to increase in afterload, impair tissue blood flow (due to vasoconstriction)
