gonorrhea

A
  • caused by neisseria gonorrhaeae aka Gonococcus, gram- diplococci. has pili-IgA protease, prevents phagocytosis by PMNs
  • common inf that causes insterility
  • NEONATAL gonococcal conjunctivities: major cause of BLINDNESS in Afr+Asia, in US, ab’s into conjunctiva
  • ACUTE, SUPPURATIVE, GENITAL TRACT INFECTION, Asx carriers are a reservoir of infection
  • males: rarely ascends, purulent urethral discharge and dysuria. UNTX: URETHRAL STRICTURE
  • females: half are asx, half endocervicitis w. vaginal discharge/bleeding –> Fallopian tubes (pus and pain)–> violin string adhesions, fitz-hugh-curtis sd (peritoneum), chronic and acute salpingitis, PID ==>infertility, chronic endometritis

chancroid

A

-genital ulcers caused by haemophilus ducreyi, gram - rods (chain-like in tissue).
-enters via break in skin –> ulcerates –> LN (may pus). 7-9d after 1st lesion appears, half pts get unilateral, painful, pus inguinal lymphadenitis (bubo) –> pus
SX: PAINFUL ULCERATION IN GENITAL AND LYMPHADENOPHATY
TX: ERYTHROMYCIN

granuloma inguinale

A
  • caused by calymmatobacterium granulomatis (small encapped, nonmotile gram - rod), tropical ulcerating dz common in tropics in 15-40yrs
  • characteristic lesion: raised, beefy-red, soft, superficial genital ulcer, inguinal and perianal regions. (can cause elephantiasis from obst lymphatics). lesion is infiltrated by macrophages, PMNs, plasma, lymphocytes. has DONOVAN BODIES = bacteria clustered around macrophage.
  • 2ary fusospirochetal infection: ulceration, mutilation, genital amputation
