sporotrichosis
- caused by inoculation by sporothrix schenckii, a mold in soil and decaying plants. common in gardeners.
- chronic dermal/subcut infection and spreads along lymphatic channels (like chains), if spread extracutaneous –> joints and bone most common (TX antifungal). (ankle, wrist, knees): begins as solitary nodule –> weeks later more nodules appear along the lymphatic drainage site. Nodules ulcerate, drain serosanguineous fluid.
-SX: SKIN LESIONS: granulomatous on periphery and center suppurative with surr exuberant pseudoepithelomatous hyperplasia. TX: iodine therapy
-some yeasts are surr by eosinophils –> ASTEROID BODY
with SPLENDORE-HOEPPLI SUBSTANCE surrounding (probably ag-ab complexes

- caused by several fungal species.
- “copper pennies” bc fungi are brown, round, thick walled
- common in barefoot agricultural workers in tropic areas.
- papules –> years later comes verrucous, crusted, ulcerated, and spread via lymphatics and involves whole limb.
dermatophyte infections
-resident in soil, animals, humans, spread by direct contact. -localized infection of keratinized tissue (skin, hair, nails).
ringworm/tinae: tinea capitis - scalp; tinea pedia - feet = atheletes feet; tinea unguium - nails; tina cruris - groin intertriginous area (jock itch)
most common minor skin dz in medical for help. TX: antifungals
mycetoma
-common in barefoot farmers and laborers (macerations facilitate inoculation with soil organisms), madura foot
SX: painless, localized swelling that expands and makes sinus tracts of lesions that eventually drains thru skin. the expansion goes to bone and surr structures –> suppurative and granulomatous rxn –> disfigurement.
-colonies of organisms called “grains” resemble “sulfur granules” of actinomycosis. TX: wide excision/amputation
