What is sarcoidosis?
It is a multi system disorder, chronic non – caseating granulomatous disorder of unknown origin. Within the lungs, this chronic inflammation results in fibrosis.
What is the general epidemiology of sarcoidosis?
It usually affects adults aged 20 – 40 and is more common in afro-Caribbean’s and women
What is the presentation of respiratory sarcoidosis?
Dry cough, progressive SOB, reduced exercise tolerance, chest pain
What is the presentation of GI sarcoidosis?
Hepato-splenomegaly
What is the presentation of ophthalmological sarcoidosis?
Conjunctivitis, dry eyes, glaucoma
What is the presentation of neurological sarcoidosis?
Bells palsy, neuropathy
What is the presentation of metabolic sarcoidosis?
Hypercalcemia
What is the general presentation of sarcoidosis?
Lymphadenopathy, polyarthritis, erythema nodosum
What investigations can be used to confirm a diagnosis of sarcoidosis?
Bloods, spirometry, DCLO, CXR, urine
What blood results would be indicative of sarcoidosis?
Raised ESR, WCC, serum ACE and calcium
What spirometry results would be indicative of sarcoidosis?
Normal restrictive pattern (Decreased FVC and FEV1 but normal FER)
What DCLO results would be indicative of sarcoidosis?
Normal or reduced
What CXR results would be indicative of sarcoidosis?
Bilateral hilar lymphadenopathy, fibrosis
What urine results would be indicative of sarcoidosis?
Increased Calcium
What are the indications that treatment is required for lung sarcoidosis?
Interstitial lung disease, hypercalcemia and cardiac/neurological involvement / uveitis
