System through which NHS organisations are accountable for continuously improving quality of their services and safeguarding high standards of care by creating environment in which excellent care will flourish

What are the components of clinical effectiveness?
Guidelines, clinical audit, outcome measures, health economics, research and development, evidence based practice
What are the components of adverse events and clinical risk management?
Adverse event review, continuity plan, risk management, patient safety, regulating compliance
What are the components of continuous improvement?
Evaluation, education and training, improvement programmes, collaberatives
What are the components of person centeredness?
Volunteering, patient experience, communications and engagement, participation, feedback, equality and diversity
What are some triggers for risk reporting?
Maternal death, intrapartum stillbirth, admission to ITU, shoulder dystocia, APGAR<7, unable to transfer to labour suite, staff shortage, near miss
Why do people risk report?
Don’t want it to happen again, improve services for everyone, identify operational issues, ensure safe practice is occurring, protect public, learn from mistakes
