find this person’s risk of having a fracture in the next 10 years
age female weight height previous fracture smoking now parent had a hip fracture glucocorticoids rheumatoid arthritis osteoporosis more than 3 units a day
what would you advise someone who was at high risk of having a fracture?
stop smoking drink less maybe take NSAIDs instead lose weight take calcium and it D do weight bearing activities avoid under nutrition
what would you use to assess stroke risk after a TIA?
ABCD2
age over 60
Blood pressure over 140/90
Clinical features (unilateral weakness; speech disturbance)
Duration of symptoms (over 10 min or over 1 hour)
Diabetes
what would you do for someone with a low risk ABCD2 score?
consider imaging and lifestyle changes so daily physical activity; low salt, low sat fat, high fruit and veg, stop smoking, low alcohol
what would you do for someone with a high ABCD2 score?
imaging, lifestyle changes so low sat fat, low salt, high fruit and veg, daily physical activity, alcohol and smoking also consider anti HTN, lipid modification, anti platelet, anticoag
Assess this person for risk of having a DVT?
Active cancer (treatment in last 6 months) recently bedridden calf swelling more than 3 cm or entire leg swollen non varicose superficial veins pitting oedema in symptomatic leg paralysis paresis plaster immobolisation recently previous DVT tenderness
how would you prevent a DVT?
hydration mobilisation low alcohol LMWH compression stockings
assess this person’s risk of cardiovascular disease
age male Pakistani, Bangladeshi, Indian Postcode Smoking diabetes angina/MI in 1st degree relative CKD 4/5 on anti hypertensives RA AF cholesterol or HDL ratio systolic blood pressure high high BMI
