What are the two main factors when conducting a risk assessment?

A
  1. Likelihood of the risk
  2. Severity of the risk

A risk assessment matrix can be used to determine the level of risk and whether controls measures need to be put in place.

What are the four levels of risk assessment and what do they mean?

A

Level 1: Generic Risk Assessment

  • Conducted by the company
  • Ensures there are appropriate safeguards in SMS in the form of policy, procedures, work instructions

Level 2: Task-Based Risk Assessment

  • Task-specific risk assessments
  • Risk assessments carried out by those involved which PTW will then be created for

Level 3: Toolbox Talk

  • Supports TBRA
  • Conducted prior to work outlines the job in hand and what hazards are involved

Level 4: Personel Risk Assessment

  • Informal risk assessment conducted all the time automatically by people
  • If there are any concerns then work should be stopped and the risk re-assessed

  • Electrical hazardous work
  • Working on machinery, lifts and lift trunks

What would you expect to find in a permit to work?

A
  • Description of Job
  • People involved
  • Location of work
  • Time (Can be no longer than 24hrs)
  • Appropriate equipment is being used and has been checked
  • Potential risks and hazards have been reviewed
  • Signed by a responsible officer, work team leader and Captain or Chief Officer prior to the job and signed on completion of work too.
