what causes cor pulmonale?
right sided heart failure to primary pulmonary disorder
absence of left sided heart dx
common causes of cor pulmonale?
parenchyma dx and chronic hypoxia
such as: COPD (most common), Cystic fibrosis, OSA, and interstitial lung dx
Pulmonary vascular dx: pulmonary embolism
symptoms of cor pulmonale:
dyspnea, fatigue, on exertion, exertional angina (due to decreased cardiac output) exertional syncope (due to high cardiac demand)
What is seen on physical exam with Cor pulmonale?
JVD
peripheral edema
palpable RV heave and loud S2 and tricuspid regurg murmur. see hepatosplenomegly with pulsatile liver
what labs are done and what is shown on EKG, TTE and catherization
EKG- see complete or incomplete right bundle branch block
see R axis deviation, RV hypertrophy and RV enlargement
TTE: pulmonary HTN, RV dilation/dysfunction, tricupid regurg
Catherization: shows elevated filling pressures and decreased cardiac output and pulmonary HTN
