for older pts who have heat stroke treatment is:
supportive measures like: airway protection circulatory support
rapid evaporative cooling
cold IV normal saline boluses for hypotension or volume depletion
cooled oxygen blankets
temperature monitoring via rectal or esophageal probes
what is evaporated cooling?
safe non invasive and effective
doesn’t interfere with other treatment or monitoring
sprays mists of water with fans and blowing over moist skin
why do we not like ice water baths for nonexertional older pts who have heat stroke?
because of increased mortality in elderly
younger pts who have exertional heat stroke tolerate this better.
do we give antipyretics for heat stroke?
no because it’s not regulated by the hypothalamus
aspirin can exacerbate complications like renal failure or DIC
is dantrolene helpful for heatstroke?
no only meant for neuroleptic malignant syndrome
